Product Giveaway Terms and Conditions

1.    Eligibility: This giveaway is open to licensed veterinarians who are legal residents of the United States, aged 18 and above, excluding employees and immediate family members of PetVivo Holdings, Inc.

2.    Entry: To qualify for a free dose of Spryng with OsteoCushion Technology, participants must be licensed veterinarians, have previously purchased Spryng, and complete and submit the 2023 Feedback survey provided by Petvivo Holdings, Inc. Surveys must be submitted in their entirety to be eligible for the free dose.

3.    Limitations: Each eligible participant is entitled to one free dose per account or practice.

4.    Duration: The giveaway begins on October 24, 2023 11:00 a.m. CT, and ends on November 10, 2023, 11:59p.m. CT.  Surveys received after the closing date will not be considered.

5.    Sample Distribution: All eligible and vetted participants will receive a free dose of Spryng. No cash or other alternatives will be offered.

6.    Privacy: All information collected through the survey will be used solely for the purposes of this giveaway and will not be shared with third parties.

7.    Publicity: By participating, you consent to Petvivo Holdings, Inc. using your survey responses for promotional purposes without additional compensation.

8.    Expiration Date: This offer expires on Nov 10, 2023, at 11:59pm CT.
